Your Role in Patient Care:

As a Patient Care Manager, you'll be responsible for supervising and coordinating clinical services, providing and directing nursing care to patients in their homes according to physician orders and regulations.

What You'll Be Doing:

Provide clinical services within your scope of practice, following the plan of care and collaborating with the healthcare team.
Receive referrals, assign clinicians, evaluate patient orders, and schedule start-of-care visits.
Coordinate the determination of patient home health benefits, medical necessity, and insurance approvals.
Ensure patient needs are continually assessed, and care is individualized, appropriate, and meets eligibility criteria and physician orders.
Review assessments and care plans daily, providing recommendations to clinicians as needed.
Work a hybrid schedule, with some days in the office, and some days in the field.
What You'll Need:

Current RN licensure in the state of practice.
Current CPR certification.
Current Driver's License, vehicle insurance, and access to reliable transportation.
Employment Type: Full-Time
Salary: $ 78,082.00 Per Year
