Job Title

Home Health RN (Field / In-Home Care)

Job Summary

We are seeking a dedicated Registered Nurse (RN) to deliver high-quality in-home patient care, coordinate interdisciplinary care, and oversee the plan of care for home health patients. Charting must be completed on-site using Homecare Homebase (HCHB) and OASIS. This is a field role requiring travel within an assigned territory.

Key Responsibilities

• Perform nursing assessments, interventions, and patient/family education in the home setting
• Develop, implement, revise, and evaluate the Plan of Care in collaboration with the interdisciplinary team
• Conduct wound care, medication administration, and other skilled nursing services per orders
• Provide care coordination: liaise with physicians, therapists, social workers, and other providers
• Complete all required documentation (HCHB, OASIS) while at the patient’s home
• Monitor patients’ progress, identify changes, and escalate concerns appropriately
• Participate in on-call rotation (1 night/week or as assigned)
• Provide weekend coverage as needed
• Travel up to 50 miles from home base; manage daily patient routing
• Maintain professional licensure, certifications, and comply with agency policies

Workload & Productivity

• Average caseload: 4–6 patients per day
• Weekly productivity: 24–30 points (or equivalent system metric)
• Guaranteed hours vary by facility; schedule will be discussed during interview

Qualifications / Requirements

• Active RN license (must have valid license in hand at time of hire; pending license not accepted)
• Minimum 1 year home health experience , plus 2 years overall clinical experience
• Proficient in HCHB and OASIS charting
• Basic Life Support (BLS) certification (required)
• Ability to drive and willingness to travel up to 50 miles, with mileage reimbursed at IRS standard
• Local clinicians within 50-mile radius are eligible
• Strong clinical judgment, organizational skills, and communication skills
